Preliminary tour date(s) availability may be noted.
2. Care
network
Information is assessed and applied to the PCHF Care Residence Network.
All elements of the transitioning senior's needs are matched with care residences & caregivers in our network, ensuring the environment fits your senior loved one's needs.
Two to three care residences will be identified that best match your senior loved one's needs.
3. CARE
TourS
The care residences and their details are reviewed with you and may include facility photos.
Tour dates, times and addresses are confirmed. PCHF will be meet you at the first location & PCH will remain with you through all tours.
You have the opportunity to decide which care residence best meets your senior loved one's needs. The choice is yours. You will not be pressured for a decision.
